End-of-Life Compass: Specialized support for those navigating the sacred transition of end-of-life Focus on living with comfort, dignity, and legacy, bridging the gap between medical needs and spiritual peace.
1. Holistic Nursing in Home Visit - $125 plus travel*
Physical & Environmental Assessment: A head-to-toe nursing assessment and home safety evaluation.
Medication Review & Reconciliation: Review and evaluation of all prescriptions, supplements, and OTC medications to ensure safety, identify potential interactions, and simplify your routine.
Also Included: Medication planning, wound care and dressing changes, line and drain management
2. Concierge Nursing Service- $500 per month (includes triage availability plus a monthly comprehensive nursing assessment/ visit.)
On-demand professional insight when you need it most.
Triage & Advice: Direct phone access for "in-the-moment" health concerns. Whether you’re unsure if a symptom warrants an ER visit or you need immediate guidance on managing a flare-up, I am a phone call away. If unable to answer, I will return the call within an hour. For emergencies call 911.
4. Care Coordination & Health Advocacy - $200 plus travel*
Navigating the modern healthcare system can be overwhelming. I act as your professional voice and "translator."
Appointment Advocacy: Based on availability. I can attend doctor visits with you to ensure your questions are answered and your wishes are heard.
Provider Liaison: I communicate across your various specialists and primary care providers to ensure your care plan is cohesive, preventing "siloed" medicine.
